Child Identification Sheet
THE FOLLOWING CHILD IDENTIFICATION INFORMATION IS BEING For the last few years, at the Orange County Fair, the Orange County Sheriff’s Department and Drug Use Is Life Abuse has staffed a booth providing parents with a “Child Identification Sheet”. This sheet is now being made available to all citizens via our website. With the advancement in DNA technology, fingerprinting children is no longer the method of choice for child identification. Please print the following sheet, fill out the information requested, and follow the directions for collecting a DNA sample from your child. Once complete, place the DNA sample into a cotton/paper envelope and place that envelope and the completed sheet with a current photograph into a freezer safe storage bag. Place this bag inside your freezer for safekeeping. If your child ever becomes lost or is abducted you will have a current photo and all necessary information to give to the police. Update the photo and personal information as directed on the sheet. The Sheriff’s Department does not keep your child’s personal information on file (this includes fingerprints and DNA). DO NOT return the completed forms to us they are for you to keep. Feel free to copy and distribute this sheet to your family, friends and neighbors. Thank you.
